This document consists of three leaves measuring 12⅝ × 7⅞ inches (32 × 20 cm). One foolscap sheet was folded in half to make two leaves and then stitched to a foolscap sheet that had previously been cut in half. Various notations and equations are written in black ink on the recto of the third leaf, including an endorsement in unidentified handwriting that reads “s | Nov 2 1831”. This endorsement possibly identifies either the location where the document was copied or the intended owner of this document. A further endorsement on the verso of this leaf, written in graphite by , reads “An appendix to Revelation”. The lower portions of the second and third leaves contain a large ink spot.

Historical Introduction
Dated 3 November 1831, this revelation is a loose document that was inserted into Revelation Book 1 at an unknown time, thereby becoming associated with the manuscript book though not physically part of it. The paper used to record this revelation does not match the paper in the manuscript book, eliminating the possibility that missing pages from Revelation Book 1 were used to copy this document.
Textual comparison of this document with other copies of the same revelation confirms that although this document is an early copy, it does not appear to be an earlier version than the one copied into Revelation Book 1 . This revelation was first printed in the 1835 Doctrine and Covenants.
